Notice of Privacy Practices
Dimensions Healthcare System is committed to protecting the privacy and confidentiality of our patient’s health information. The HIPAA Privacy Rule is the federal law designed to help protect the privacy of patient’s health information. “HIPAA” stands for the Health Insurance Portability and Accountability Act of 1996.
The HIPAA Privacy Rule requires that we give each patient our Notice of Privacy Practices to explain how we may use their health information. Our privacy practices will be followed by all Dimensions Healthcare System facilities including Bowie Health Campus, Laurel Regional Hospital, Prince George’s Hospital Center, and the Senior Health Center.

Request a Copy of Your Medical Records
If you would like to request a copy of your medical record, you must complete an Authorization for Release of Protected Health Information. If you are the legal guardian, healthcare agent, power or attorney or other legal representative of the patient, please attach proof of your authority to act on behalf of the patient. Please be sure to fill-in the form completely, including signature and date, or the form may be returned to you for completion. Please submit completed forms by mail, fax (the fax request will only be accepted for urgent, continuity of care requests to physician offices, or other urgent care settings) or in person, to:

Medical Records FAQs
1. Will you mail the copies or will I have to pick them up?
We can mail the copies to the address you specify on the Authorization form, or you can make arrangements to pick them up during our hours of operation.
2. Will you fax medical information?
We will only fax medical records to another healthcare provider or facility for continuing medical care.
3. Is there a fee for copying medical records? Yes, there is a fee to copy medical records. $0.76 per page copy charge. $22.88 preparation charge (This fee is not charged to patients.) There is no charge for records sent directly to a physician or healthcare facility for continuing care. The fees will be in compliance with applicable Maryland State guidelines.

Request a Copy of Your Birth or Death Certificate
